A short letter is needed to be written for the purpose of Requesting a Medical Leave of Absence, I am a new employee, less than 30 days, and do not have any benefits package with the company. I will be going for 2 /3 surgeries and I do not know what time I will be able to get back to work. therefore I do not have a return date, which is pending when am fit to work ( i wish not to return soon anyway)
