- Anevaluation of the health needs of the group and a proposed continuum ofcare level (preventive, treatment, or long-term care) based on the group’sissues, risk factors, and needs. Justify the proposed level withsupportive research/evidence.
- Adescription of two proposed services your program will include.
Yourassignment should be a minimum of four pages in length (excluding title and reference pages), and should include a minimumof four scholarly sources cited according to APA guidelines.
